Patients under 18 must apply through Colorado’s minor-patient process with a parent or legal guardian.After your evaluation with Dr. Cohen, you must submit your own online application to the State of Colorado Medical Marijuana Registry with proof of residency and the required state fee.

Getting your MMJ Card involves two separate steps: a medical evaluation with Dr. Cohen and an application to the State of Colorado Medical Marijuana Registry.Complete your evaluation with Dr. Cohen. Dr. Cohen reviews your medical history, provides a personalized plan, and electronically submits your Provider Certification to the State of Colorado Medical Marijuana Registry. This Provider Certification is required before you can apply for your MMJ Card with the Registry. Dr. Cohen submits your Provider Certification. Right after your appointment, Dr. Cohen electronically submits a Provider Certification to the State Registry. This certification is not your MMJ Card. Complete your State Registry application. You must log in to your Registry account, complete the required online application, upload a valid Colorado ID, and pay the State’s nonrefundable application fee. New patients must create an account; renewal patients should use their existing account. The Registry reviews your application. The State Registry—not Dr. Cohen or Holos Health—reviews your application. If approved, the Registry issues your MMJ Card, which you can download or print from your account with them.Important: Holos Health cannot access or complete your State Registry application for you. Dr. Cohen’s appointment fee and the State’s application fee are separate.

The Provider Certification is the document required by the State of Colorado to be able to apply for a MMJ Card with the Registry.You must then complete a separate application with the State of Colorado Medical Marijuana Registry.
