Service Description

Holos Health has extensive experience supporting qualifying patients under age 21 and their families, including patients with serious conditions such as cancer, autism spectrum disorder, and seizure disorders. During this online evaluation, Dr. Joe Cohen will review the patient’s medical records, health history, medications, qualifying condition, and questions. He will discuss the potential role, risks, and practical considerations of medical cannabis or CBD and provide individualized guidance. After your appointment, Dr. Cohen will electronically submit the Provider Certification to the Colorado Medical Marijuana Registry. The Registry—not Dr. Cohen or Holos Health—reviews the completed State application and issues the MMJ Card. **MEDICAL RECORD REQUIREMENTS** During the online booking process you will be prompted to upload current records from the physician, psychiatrist, or other treating provider who diagnosed the qualifying condition. The records must clearly state the patient’s diagnosis or qualifying medical condition. If the qualifying condition is a mental health condition diagnosed by a psychiatrist, Holos Health also requires a letter from the treating psychiatrist supporting the consideration of medical cannabis. **PATIENTS AGES 18–20** Patients who did not hold a Registry card before age 18 and first applied on or after January 1, 2022, must obtain two Provider Certifications each year. - The certifications must come from providers at two different medical practices unless the patient is homebound. - This appointment provides one certification. A separate evaluation is required for the second. **PATIENTS UNDER 18** Colorado requires two Provider Certifications for patients under age 18. - The certifications may come from providers within the same medical practice. - This appointment provides one certification. A separate evaluation is required for the second. - A parent or legal guardian must complete the State application and required consent and caregiver documentation. - Contact Holos Health if you need possible referrals for a second provider.
