New and Renewal Patients | Over 21 Patients
Service Description
This online appointment is designed for new patients and returning patients seeking a more comprehensive evaluation, additional education, or personalized medical cannabis guidance. During your visit, Dr. Joe Cohen will review your health history, medications, qualifying condition, cannabis-related questions, and goals. He will provide individualized education and thoughtful recommendations, including any clinically appropriate changes to your certification or care plan. After your appointment, Dr. Cohen will electronically submit a Provider Certification to your Colorado Medical Marijuana Registry account. You must then complete the Registry’s separate online application and pay the State application fee. The Registry—not Dr. Cohen or Holos Health—reviews your application and issues your MMJ Card. In-person evaluations are preferred whenever possible because they provide the most comprehensive care experience. This online option is available when health concerns, distance, transportation, or other circumstances make an office visit difficult.
Cancellation Policy
To cancel or reschedule, please reschedule or contact us 24 hours in advance. While we understand situations arise, we value your time, the doctor's time, and those waiting to see the doctor. As such, No-Show Appointments are not eligible for a refund and/or will be assessed a $50 fee. Rescheduled appointments that are rescheduled on the day of an appointment will be assessed a $50 fee. If you are late for your appointment, we will do our best to accommodate your needs with the remaining time left in your appointment, but can not guarantee that we can see you for your full appointment time.
