Dr. Joe Cohen offers 30-minute, in-person MMJ evaluations for new and renewal patients at his private Gunbarrel home office. This comfortable, confidential setting allows time for a comprehensive and personalized conversation. During your appointment, Dr. Cohen will review your health history, medications, qualifying condition, cannabis-related questions, and goals. He will provide individualized education and thoughtful recommendations, including any appropriate changes to your certification or care plan. After your appointment, Dr. Cohen will electronically submit the Provider Certification to the Colorado Medical Marijuana Registry. You must then complete the Registry’s separate online application and pay the applicable State fee. The Registry—not Dr. Cohen or Holos Health—reviews the completed State application and issues the MMJ Card. **COMPLIMENTARY FUNCTIONAL MUSHROOM TINCTURE** Your Gunbarrel appointment includes the option to receive a complimentary 1-ounce Clinically Informed functional mushroom tincture, while supplies are available.
