These Terms and Conditions govern your access to and use of the Holos Health website at www.holoshealth.org, including its pages, forms, online booking features, communications, and related services. The website is operated by Jocodo LLC, doing business as Holos Health (collectively, “Holos Health,” “we,” “us,” or “our”).

By accessing the website, submitting a form, or booking an appointment, you agree to these Terms and Conditions and acknowledge our Privacy Policy. If you do not agree, do not use the website.

About Holos Health

Holos Health is a physician consultation practice led by Joseph Cohen, DO. Services offered through the website may include Colorado medical marijuana evaluations and educational Micro-Dose Consultations. Appointment formats, eligibility requirements, locations, prices, durations, and availability are identified in the applicable booking description and may change.

The website provides general information about Holos Health and allows visitors to learn about services, contact the practice, and request or schedule appointments. It is not an online pharmacy, dispensary, product marketplace, or emergency medical service.

​

No Cannabis or Micro-Dose Product Sales

Holos Health is not a cannabis dispensary, psychedelic dispensary, pharmacy, product retailer, manufacturer, or product-referral service.

​

Holos Health does not sell, prescribe, or manufacture access to cannabis, marijuana, psilocybin, psychedelic substances, capsules, or other micro-dose products.

​

Appointment fees are solely for professional services. Depending on the appointment, those services may include Dr. Cohen’s time, review of health history and medications, medical evaluation, individualized education, a protocol, and, when clinically appropriate, a Colorado Medical Marijuana Provider Certification.

General Website Information

Content on this website is provided for general educational and informational purposes. Reading the website, completing a contact form, subscribing to communications, or creating a booking request does not by itself establish a physician-patient relationship.

A physician-patient relationship is established only when Holos Health accepts you as a patient and Dr. Cohen provides professional services within an appropriate appointment or course of care. General website content is not individualized medical advice and should not be used to diagnose or treat a condition or to replace care from a qualified healthcare professional.

​

Information about cannabis, cannabinoids, micro-dosing, supplements, or emerging research is not a promise or guarantee of safety, appropriateness, legality, or a particular outcome. Research may be incomplete or change over time. Individual results vary.

Medical Emergencies and Urgent Concerns

The website, contact form, email, text number, and online booking system are not monitored as emergency services. Do not use them for urgent medical or psychiatric concerns.

​

If you are experiencing a medical or mental health emergency, call 911 or go to the nearest emergency department. If you are experiencing thoughts of suicide or self-harm, call or text 988 in the United States or seek immediate local assistance.

​

Colorado Medical Marijuana Evaluations

Colorado medical marijuana evaluation services are available only to patients who satisfy current Colorado requirements. Eligibility depends on the patient’s residence, age, qualifying condition, documentation, and other requirements established by Colorado law and the Colorado Medical Marijuana Registry.

Dr. Cohen makes an independent clinical decision during each evaluation. Booking and paying for an appointment do not guarantee that Dr. Cohen will issue a Provider Certification, Uniform Certification Form, extended plant count, higher purchase amount, or any other certification or recommendation.

A Provider Certification is not an MMJ Card. When a Provider Certification is issued, the patient must separately apply through the Colorado Medical Marijuana Registry, provide the documents requested by the Registry, and pay the separate State application fee. The Registry, not Dr. Cohen or Holos Health, reviews the application and issues or denies the MMJ Card. Holos Health cannot access a patient’s Registry password, payment account, or completed State application on the patient’s behalf.

Patients under 21 are subject to additional State requirements, which may include additional provider certifications, medical records, and participation by a parent or legal guardian. A parent or legal guardian must handle website and booking activity for a patient under 13.

​

In-person evaluations are preferred. Limited online MMJ appointments may be offered when clinically and legally appropriate, including circumstances involving medical, geographic, transportation, or comparable hardship. Holos Health may require an in-person evaluation or decline an online appointment.

​

Micro-Dose Consultations

Micro-Dose Consultations are educational physician consultations for adults age 21 and older. They may include review of health history, medications, goals, potential risks or contraindications, research, and an individualized educational protocol appropriate to the selected visit.

​

Micro-Dose Consultations do not include a prescription or seller referral. Holos Health does not authorize unlawful possession, purchase, cultivation, manufacture, or use of a controlled substance.

​

Micro-dosing is an area of emerging research. A consultation does not establish that micro-dosing is safe or appropriate for every person and does not guarantee a medical, psychological, or wellness outcome. Dr. Cohen may recommend that a patient not pursue micro-dosing or seek another form of care.

​

Laws concerning psilocybin and other psychedelic substances vary by jurisdiction and may change. You are responsible for understanding and complying with the laws that apply where you are located. Nothing on this website is legal advice.

​

Telehealth and Online Appointments

Online services depend on the type of appointment, the patient’s physical location, applicable professional licensing rules, and other legal or clinical requirements. You must provide your accurate physical location at the time of an online appointment and notify Holos Health if that location changes.

​

Telehealth has limitations. Technology can fail, communications can be delayed, and a remote examination may not provide the same information as an in-person examination. Dr. Cohen may pause or end an online appointment, request additional documentation, recommend in-person evaluation, or refer you to local or emergency care when appropriate.

​

You are responsible for having a private setting, compatible device, reliable internet connection, and accurate contact information. Do not record an appointment without the prior consent of every participant.

​

Appointments, Fees, Cancellations, and Refunds

Current prices, appointment durations, locations, and availability appear in the booking system. You agree to review the complete service description before booking and to select the service, location, format, and patient category that apply to you.

​

Payment requirements and the cancellation, rescheduling, no-show, and refund policy presented during booking are incorporated into these Terms. If a service-specific policy conflicts with a general statement on the website, the policy displayed and accepted during booking controls for that appointment.

​

Holos Health may correct pricing or scheduling errors, request additional information, decline or cancel an appointment, or require a different appointment type. If Holos Health cancels an appointment and does not provide the purchased service, any refund will be handled according to the applicable booking and payment policy.

​

Fees paid to Holos Health do not include the Colorado Medical Marijuana Registry application fee, dispensary purchases, products, prescriptions, laboratory services, outside professional services, or any other third-party charge unless expressly stated in the booking description.

​

Information You Provide

You agree to provide complete and accurate information when using the website or booking services. You may not impersonate another person, submit information without authorization, or use another person’s payment method without permission.

​

Do not place medical records, detailed medical histories, Social Security numbers, government identification, Registry login credentials, payment-card numbers, or other sensitive information in the general website contact form, ordinary email, or text messages. Current patients should use the designated secure patient system for clinical questions, records, and sensitive health information.

​

Electronic Communications

By providing contact information in connection with an inquiry or appointment, you authorize Holos Health and its service providers to send service-related communications by email, telephone, or text, such as booking confirmations, reminders, schedule changes, requests for information, and responses to your inquiry.

Email and standard text messaging may not be fully secure. Avoid sending sensitive medical information through those channels unless Holos Health has specifically directed you to an approved secure method.

Marketing messages will be sent only as permitted by law and according to the preferences you provide. You may unsubscribe from marketing emails using the link in the message. If you separately consent to recurring marketing text messages, you may opt out by replying STOP. Opting out of marketing does not prevent necessary service-related communications about an existing appointment or patient relationship.

Privacy and Medical Information

Our collection and use of website information are described in the Holos Health Privacy Policy. Medical information created or received as part of patient care may also be governed by applicable health privacy law and a separate Notice of Privacy Practices.

The general website Privacy Policy does not replace any Notice of Privacy Practices that applies to protected health information. Requests involving medical records or rights under an applicable Notice of Privacy Practices must be directed through the contact method identified in that notice or the secure patient system.

Third-Party Platforms and Links

The website may rely on or link to third-party services, including website hosting, online booking, payment processing, video communications, maps, analytics, email or text communications, patient systems, and government websites. Those services have their own terms, privacy practices, security measures, and availability.

Holos Health does not control the Colorado Medical Marijuana Registry, dispensaries, external research databases, or other third-party websites. A link does not mean Holos Health endorses every statement, product, service, or practice offered by the third party.

Intellectual Property

Unless otherwise stated, the website and its original text, design, graphics, logos, educational materials, and other content are owned by or licensed to Jocodo LLC. They are protected by applicable intellectual property laws.

You may view and print reasonable portions of the website for personal, noncommercial use. You may not reproduce, publish, sell, modify, scrape, distribute, or create derivative works from website content without prior written permission, except as allowed by law.

Prohibited Use

You may not use the website to:

• violate a law or another person’s rights;

• obtain, advertise, sell, or facilitate access to an illegal or controlled product;

• submit false, fraudulent, threatening, harmful, or misleading information;

• interfere with the website, booking system, security features, or another user’s access;

Holos Health may restrict access, cancel a booking, or take other appropriate action when it reasonably believes the website or services are being misused.

We work to keep the website and booking tools accurate and available, but we do not guarantee uninterrupted access, error-free operation, immediate availability, or that all information will always be complete or current. Service descriptions, research summaries, pricing, schedules, laws, and third-party information may change.

To the fullest extent permitted by law, the website is provided on an “as is” and “as available” basis. Holos Health disclaims warranties concerning the website that are not expressly stated in these Terms. To the fullest extent permitted by law, Jocodo LLC and Holos Health will not be liable for indirect, incidental, special, consequential, or punitive damages arising solely from use of, or inability to use, the public website or a third-party link or platform.

This limitation does not apply where liability cannot lawfully be limited. It does not limit obligations that arise from professional medical services, applicable health privacy law, or other duties that cannot be waived by contract.

Governing Law

These Terms are governed by the laws of the State of Colorado, without regard to conflict-of-law principles. Any legal proceeding concerning the public website or these Terms must be brought in a court with jurisdiction in Boulder County, Colorado, unless applicable law requires otherwise.

When Holos Health handles protected health information as a covered healthcare provider, that information is used and disclosed in accordance with applicable law and the Holos Health Notice of Privacy Practices.

The Notice of Privacy Practices explains patient rights concerning medical records and how protected health information may be used or disclosed for treatment, payment, healthcare operations, and other purposes allowed by law.

Holos Health directs clinical messages and records to its designated secure patient system. We may still send nonmarketing messages needed to administer an appointment or respond to your request.

Holos Health does not sell personal information for money. Holos Health does not sell protected health information or medical records and does not use protected health information for targeted advertising.

Some state privacy laws define “sale,” “sharing,” or “targeted advertising” broadly enough to include certain disclosures involving analytics or advertising cookies. To the extent such laws apply and Holos Health uses technology covered by those definitions, we will provide applicable opt-out controls and process legally recognized universal opt-out signals as required.

Children and Younger Patients

The public website is intended for a general audience and is not directed to children under 13. We do not knowingly collect personal information directly from a child under 13 through the public website without appropriate parental or legal-guardian involvement.

A parent or legal guardian must submit website and booking information for a patient under 13. Colorado MMJ patients under 21 are subject to additional State requirements. Online Micro-Dose Consultations are limited to adults age 21 and older.

If you believe a child under 13 submitted personal information through the website without appropriate authorization, contact us so we can review and address the information.

Links to Other Websites

The website may link to government agencies, research databases, patient systems, maps, or other third-party websites. Holos Health does not control those websites or their privacy and security practices. Review the privacy policy of any third-party site before providing information.
