Colorado MMJ Card Application Help
Your appointment with Dr. Cohen and your State MMJ Card application are separate steps. After Dr. Cohen submits your Provider Certification, you must complete an online application with the Registry for your card, and pay the State fee through the Colorado Medical Marijuana Registry.
Holos Health cannot access your Registry account. For login, password, duplicate-account, application, payment, or status questions, contact the Colorado Medical Marijuana Registry directly.

Using the right account prevents avoidable delays before you begin the application.
Already created more than one account? Contact the Registry before starting another application.
STEP BY STEP
From Provider Certification to MMJ Card.
The State application has several parts. The most common difficulty with the Registry application is uploading the Provider Certification. You must hover over each section in the top navigation menu, then select New from the dropdown menu to add the applicable information—for example, New Provider Certification, New Cultivation, and New Payment.

Sign in and open your application.
New patients create one Registry account. Returning patients sign in to their existing account, then begin the current patient application or renewal.

Complete your patient information.
Enter your information exactly as it appears on your valid Colorado driver’s license or State ID. Upload/Update any documents the Registry requests.

Add your Provider Certification.
Hover over the Provider Certification tab in the top navigation menu, then select NEW Provider Certification from the menu dropdown. On that page, select the most recent certification submitted by Dr. Cohen, scroll to the bottom of the page and save it to your application.
CAN'T FIND YOUR PROVIDER CERTIFICATION?

Hover over the top navigation menu in your Registry application and select New PC (New Provider Certification).

Review the Provider Certification copy emailed to you. Confirm that your name, date of birth, and Social Security number are correct, as this information connects the PC to your Registry account.

If any information is incorrect, contact Holos Health so we can make any necessary corrections.

If the information is correct but the certification still does not appear, contact the Colorado Medical Marijuana Registry for assistance. Holos Health cannot access or troubleshoot your Registry account.

Add your cultivation information
Hover over Cultivation in the top navigation menu and select New Cultivation. Answer the questions about how you plan to obtain your medical cannabis, complete any applicable cultivation or caregiver information, and save the section.

Pay the State fee and submit
Hover over Payment in the top navigation menu and select New Payment. Enter your payment information, pay the separate State application fee, and submit your completed application to the Registry for review.

Check your status and download your card
The Registry typically processes online applications within 3–5 business days, although processing times may vary. When a decision is available, you will receive a general email directing you to log into and check your Registry account.
Log in and look in the Tracking Inbox: Patient Registration section. Look for the word Active beside your current registration and click on that word. On the page that opens, choose Print Card to download, save, or print your new MMJ Card.
