Service Description
This discounted online renewal appointment is available to Colorado MMJ patients who currently receive Social Security Disability Insurance (SSDI) benefits or hold an active Colorado Marijuana Enforcement Division (MED) Badge. **Proof of eligibility is required.** During the booking process, please upload one of the following through the secure intake form: - Your active Colorado MED Badge - Your current SSDI award or benefits letter MED Badge holders may verify their credential status through the Colorado MED License Verification portal: https://codor.mylicense.com/MED_Verification During your secure video appointment, Dr. Joe Cohen will review your health history, medications, qualifying condition, cannabis-related questions, and goals. He will also discuss any clinically appropriate recommendations or changes to your certification. After your appointment, Dr. Cohen will electronically submit a Provider Certification to your Colorado Medical Marijuana Registry account. You must then complete the Registry’s separate renewal application and pay the applicable State fee. The Registry—not Dr. Cohen or Holos Health—reviews your application and issues your renewed MMJ Card. In-person evaluations are preferred whenever possible because they provide the most comprehensive care experience. This online option is available when health concerns, distance, transportation, work obligations, or other circumstances make an office visit difficult.
Cancellation Policy
To cancel or reschedule, please reschedule or contact us 24 hours in advance. While we understand situations arise, we value your time, the doctor's time, and those waiting to see the doctor. As such, No-Show Appointments are not eligible for a refund and/or will be assessed a $50 fee. Rescheduled appointments that are rescheduled on the day of an appointment will be assessed a $50 fee. If you are late for your appointment, we will do our best to accommodate your needs with the remaining time left in your appointment, but can not guarantee that we can see you for your full appointment time.
