We have had the distinct privilege of working with minors diagnosed with serious chronic conditions like cancer, autism, and seizure disorders, and have seen remarkable transformations with cannabinoid medicine. Our doctors will work with you to develop a comprehensive Integrative and Cannabis/CBD treatment regimen for your child, and will make sure you have legal state access to their medicine. Current medical records must be provided to Holos Health. This medical record must be from the patient's doctor and/or psychiatrist who diagnosed the patient. The medical record must state the patient's qualifying medical condition. In addition, if the patient is diagnosed with a mental health condition by a psychiatrist, we require a letter of support for using medical cannabis from the patient's psychiatrist. PLEASE CAREFULLY READ BELOW AS THERE ARE DIFFERENT REQUIREMENTS FOR: - Patients who are between the ages of 18 - 20, versus - Patients who are under 18 years old REQUIREMENTS FOR NEW AND RENEWAL PATIENTS WHO ARE AGES 18 - 20: 1. Applicants in this age group will need TWO (2) PROVIDER CERTIFICATIONS, These Provider Certifications must be from 2 doctors who are in DIFFERENT medical practices. *If the applicant in this age group is homebound, the providers do not need to be from different medical practices. 2. This appointment can be for those who need a second doctor MMJ Card appointment, who obtained their primary Provider Certification from another clinic. 3. Medical Records (see above). REQUIREMENTS FOR NEW AND RENEWAL PATIENTS WHO ARE UNDER 18: 1. The State of Colorado requires that patients who are under 18 years of age obtain TWO (2) PROVIDER CERTIFICATIONS. These Provider Certifications can be from 2 doctors who are in the SAME medical practice. 2. If you need a Primary Doctor, please book the Under 18 MMJ Card (Primary Dr.) Appt with Dr. Lisa Rittel.
