MMJ Card Requirements

To obtain a Colorado MMJ Card, the Colorado Department of Public Health & Environment requires applicants have a Qualifying Condition, and be a resident of the state with a driver’s license or Colorado ID card.

Qualifying Conditions

According to the Colorado Department of Public Health & Environment, qualifying debilitating medical conditions include: 

  • Cachexia

  • Cancer

  • Glaucoma

  • HIV/AIDS

  • Persistent muscle spasms

  • Seizures

  • Severe nausea

  • Severe or chronic pain

Colorado also allows three qualifying disabling conditions which include the following. If you have one of these conditions, medical records are required in order to book an appt with us. 

  • Opioid Replacement Therapy

  • Autism Spectrum Disorder

  • Post-Traumatic Stress Disorder

Other Requirements

Along with having a qualifying condition, a patient must be a resident of the state with a driver’s license or identification verifying proof of address, be at least 21-years-old, provide the application fee to the CO MMJ Registry, and have a Physician’s Certification confirming qualifying conditions. Additionally, a patient must have a new physical examination every 12 months to hold a valid cannabis license.
