For both NEW and RENEWAL Patients: **IMPORTANT UPDATES FROM THE STATE OF COLORADO MMJ REGISTRY - The State of Colorado now requires that patients who are under 18 years of age obtain two Provider Certifications from two different medical practices for new or renewing card holders for anyone who registered after January 2022. *If you are a renewal MMJ Card patient who registered prior to January 2022, you are grandfathered in, and only need one Provider Certification. - We require current medical records attesting to a state qualifying condition be emailed to us. Afterwhich, our provider will review them before confirming the appointment. FOR PATIENTS REQUIRING PC'S FROM TWO SEPARATE MEDICAL PRACTICES: We will serve as your primary physician, and have a network of separate medical physicians/practices. If you need a recommendation for a second medical provider for the second Provider Certification, please let us know as we have a network of preferred physicians who specialize and have experience working with minors and cannabinoid medicine. We have had the distinct privilege of working with children diagnosed with serious chronic conditions like cancer, autism, and seizure disorders, and have seen remarkable transformations with cannabinoid medicine. Our doctors will work with you to develop a comprehensive Integrative and Cannabis/CBD treatment regimen for your child, and will make sure you have legal state access to their medicine. **Please note that in Colorado, in order for a minor child to legally use any form of CBD (whether over-the-counter or from a dispensary) and/or cannabis, they must have a medical cannabis card. UNDER 18 MMJ CARD REQUIREMENTS Please copy/paste the link below to learn about the State of Colorado MMJ Registry requirements, and to download their Minor Patient Consent Form, which you will need to have notarized before applying with the Registry.

Cancelation Policy

To cancel or reschedule, please reschedule or contact us 24 hours in advance. While we understand situations arise, we value your time, the doctor's time, and those waiting to see the doctor. As such, No-Show Appointments may not be eligible for a refund, and cancellations made on the day of your appointment may be assessed a $50 cancellation fee from your appointment cost. If you are late for your appointment, we will do our best to accommodate your needs with the remaining time left in your appointment, but can not guarantee that we can see you for your full appointment time.
