**FOR NEW AND RENEWAL PATIENTS WHO ARE AGES 18 - 21, current medical records from your doctor that state your qualifying medical condition must be emailed to Holos Health. Afterwhich, our provider will review them before confirming the appointment. We have had the distinct privilege of working with minors diagnosed with serious chronic conditions like cancer, autism, and seizure disorders, and have seen remarkable transformations with cannabinoid medicine. Our doctors will work with you to develop a comprehensive Integrative and Cannabis/CBD treatment regimen for your child, and will make sure you have legal state access to their medicine. IMPORTANT UPDATES FROM THE STATE - FOR APPLICANTS AGED 18 - 20: - Applicants in this age group that DID NOT have a MMJ Registry Card before age 18 and applied for the first time on or after Jan. 1, 2022 will need Provider Certifications from two different providers at different medical practices each year when they apply. - If the applicant in this age group is homebound, the providers do not need to be from different medical practices. - If you are an applicant in this group, make sure both of your providers enter their certifications into the Medical Marijuana Registry system. Check to make sure all of your identifying information is correct and matches on both documents. Then, get copies of the certifications from your providers. *Please note that in Colorado, in order for a minor child to legally use any form of CBD (whether over-the-counter or from a dispensary) and/or cannabis, they must have a medical cannabis card.

Cancelation Policy

To cancel or reschedule, please reschedule or contact us 24 hours in advance. While we understand situations arise, we value your time, the doctor's time, and those waiting to see the doctor. As such, No-Show Appointments may not be eligible for a refund, and cancellations made on the day of your appointment may be assessed a $50 cancellation fee from your appointment cost. If you are late for your appointment, we will do our best to accommodate your needs with the remaining time left in your appointment, but can not guarantee that we can see you for your full appointment time.
