For both NEW and RENEWAL Patients. We have had the distinct privilege of working with children diagnosed with serious chronic conditions like cancer, autism, and seizure disorders, and have seen remarkable transformations with cannabinoid medicine. Our doctors will work with you to develop a comprehensive Integrative and Cannabis/CBD treatment regimen for your child, and will make sure you have legal state access to their medicine. **IMPORTANT UPDATES FROM THE STATE OF COLORADO MMJ REGISTRY 1. The State of Colorado requires that patients who are under 18 years of age obtain two (2) Provider Certifications for new or renewing card holders. These Provider Certifications can be from 2 doctors who are in the same medical practice. 2. If you need a second doctor appointment, please book the Secondary Doctor Appt with Dr. Joe Cohen in addition to this appointment. 3. Current medical records from your doctor that state the patient's qualifying medical condition must be provided to Holos Health. You will receive an email immediately after booking this appointment where you will be able to securely upload them. Afterwhich, our doctors will review them before confirming the appointment. *In addition, if the patient is diagnosed with a mental health condition by a psychiatrist, we require a letter of support for using medical cannabis from the patient's psychiatrist. If medical records are not received, we may have to cancel the appointment as these records are a legal state requirement. Please note that in Colorado, in order for a minor child to legally use any form of CBD (whether over-the-counter or from a dispensary) and/or cannabis, they must have a medical cannabis card. Cancellation Policy
To cancel or reschedule, please reschedule or contact us 24 hours in advance. While we understand situations arise, we value your time, the doctor's time, and those waiting to see the doctor. As such, No-Show Appointments are not eligible for a refund and/or will be assessed a $50 fee. Rescheduled appointments that are rescheduled on the day of an appointment will be assessed a $50 fee. If you are late for your appointment, we will do our best to accommodate your needs with the remaining time left in your appointment, but can not guarantee that we can see you for your full appointment time.
