FOR NEW AND RENEWAL PATIENTS. MMJ Card Requirements: 1. To apply for a Medical Marijuana Card in Colorado, you must be a legal resident of Colorado. 2. Patients must be aged 21 or over in order to apply for a medical marijuana card for themselves. 3. The Colorado Department of Health and Education (CDPHE) requires that all MMJ Card holders have a qualifying condition: - A chronic or debilitating disease or medical condition - Cachexia - Cancer - Epilepsy - Glaucoma - HIV/AIDS - Multiple Sclerosis - Persistent muscle spasms - Seizures - Severe nausea - Severe pain - PTSD (Medical Records are required for all patients with this diagnosis). - An Autism Spectrum Disorder (Medical Records are required for all patients with this diagnosis). - Opioid Replacement Therapy 4. After your appointment with us, you can then apply for your medical card with the State of CO MMJ Registry. *After your appointment our doctors will electronically submit your Provider Certification to the State of Colorado MMJ Registry.
Cancellation Policy
To cancel or reschedule, please reschedule or contact us 24 hours in advance. While we understand situations arise, we value your time, the doctor's time, and those waiting to see the doctor. As such, No-Show Appointments are not eligible for a refund and/or will be assessed a $50 fee. Rescheduled appointments that are rescheduled on the day of an appointment will be assessed a $50 fee. If you are late for your appointment, we will do our best to accommodate your needs with the remaining time left in your appointment, but can not guarantee that we can see you for your full appointment time.
