Our doctors will work with you to develop a comprehensive Integrative and Cannabis/CBD treatment regimen for your child, and will make sure you have legal state access to their medicine. FOR NEW AND RENEWAL PATIENTS WHO ARE AGES 18 - 20 1. Applicants in this age group that DID NOT have a MMJ Registry Card before age 18 and applied for the first time on or after Jan. 1, 2022 will need Provider Certifications from TWO (2) DIFFERENT PROVIDERS at DIFFERENT MEDICAL PRACTICES each year when they apply. *If the applicant in this age group is homebound, the providers do not need to be from different medical practices. 2. Current medical records from your doctor that state the patient's qualifying medical condition must be provided to Holos Health. You will receive an email immediately after booking this appointment where you will be able to securely upload them. Afterwhich, our doctors will review them before confirming the appointment. In addition, if the patient is diagnosed with a mental health condition by a psychiatrist, we require a letter of support for using medical cannabis from the patient's psychiatrist. *If medical records are not received, we may have to cancel the appointment as these records are a legal state requirement. *Please note that in Colorado, in order for a minor child to legally use any form of CBD (whether over-the-counter or from a dispensary) and/or cannabis, they must have a medical cannabis card.
